A FREE BOTTLE FROM ANA

We are running a program to provide our remedies for free to low income individuals and families. Eligibility for this program is determined on a case-by-case basis using the general standards of low income and poverty levels. Persons under 18 years of age may only submit requests through a parent or guardian, and should not provide personal information of the minor. As described below, we do request some kind of proof of eligibility (low income, poverty or anything similar) for this program. Submitting Your Request for a FREE Bottle:

1- Please photocopy your Medicaid card, your “Food Stamps” card or anything similar to show the same level of income.

2- Using a black pen or marker please mask out on the photocopy any claim/eligibility number, Social Security number, medical information and any other information you wish to keep private.

3- Please mail the photocopy to the address below or fax it to (408) 416 0240.
You can also scan it and send it by E-Mail.
